I'm assuming it was an underground test in Nevada, which would explain the debris falling from the cloud, but when was the test and how was it filmed?

That clip is from Atomic Filmakers by VCE.com. It was one of the overground Nevada tests in the 50's I believe. What you are seeing is a 16mm Gun Sight Aiming Point Gun Camera used during WW2 to record aerial combat, being blown off it's mounting post by the blast wave from the test explosion. GSAPs were used because they small, rugged and electrically trigger-able making them very well suited to capturing tests. Their durability is demonstrated in that clip, as being blown off its mount and spun through the air didn't stop it filming, or protecting the film stock.
